Fees & Billing

**Dr. Choulet has limited availability for new patients at this time.**

Dr. Choulet maintains a smaller practice to ensure that a high level of availability and care coordination is provided to all of her patients.

Concierge serivces are provided through a non-retainer and fee-for-service model. Additional support between appointments, such as coordination of care or professional services, i available as needed (see below: other professional services).

If Dr. Choulet's fees are outside your budget or if she is unavailable, we encourage you to request a consultation at Choulet Performance Psychiatry^®, where one of Dr. Choulet's highly qualified colleagues may be available to see you.

New Patient Consultations:
The first appointment serves as a consultation and does not guarantee a treatment agreement.

• Adult Consultation (60min) | $750
• Child & Adolescent Consultation (90min) | $850

Follow Up Appointments:

• 30min Follow Up | $375
• 50min Follow Up | $625

Other Professional Services: For services requiring longer than 5 minutes such as report/letter writing, secure portal messaging, telephone/text conversations, email reading or writing, preparation of treatment summaries, communication or coordination of care with family members, or other providers, or time spent performing any other services on a patient's behalf (i.e. reviewing psychological testing reports), charges are applied in 5-minute increments.

​​​​​​​Please note that Dr. Choulet is a fee-for-service physician and is not contracted with any insurance companies. A superbill can be provided for possible out-of-network reimbursement from the patient's insurance company.
