**Dr. Choulet has limited availability for new patients at this time.**

Dr. Choulet maintains a smaller practice to ensure that a high level of care coordination is provided to all of her patients. Concierge serivces are provided through a non-retainer and fee-for-service model. Dr. Choulet provides additional services between appointments, as needed (see below: other professional services). Please note that Dr. Choulet is a fee-for-service physician and is not contracted with any insurance companies. A superbill can be provided for possible out-of-network reimbursement from the patient's insurance company.

New Patient Consultations:
The first appointment serves as a consultation and does not guarantee a treatment agreement.

• Adult Consultation (60min) | $550
• Child & Adolescent Consultation (90min) | $750

Follow Up Appointments:

• 30min Follow Up | $300
• 50min Follow Up | $450

Any other professional services that require longer than 5 minutes such as report/letter writing, secure portal messaging, telephone/text conversations, email reading or writing, preparation of treatment summaries, communication or coordination of care with family members, or other providers, or time spent performing any other services on a patient's behalf (i.e. reviewing psychological testing reports) will be charged for each 5 minute increment.
